It is extremely important for you to call Caterpillar Benefits at 1-800-447-6434 or 309-494-2363, [Option 4, Sub-option 7] to report that you are leaving on an extended vacation.

 

v     When you are staying in one place a month or more (snowbirds, i.e.) it is your responsibility to report a change of address to Caterpillar so that your medical claims can be processed and paid correctly.  When out of the Peoria NetWork, be sure to know who your United Healthcare providers are for your vacationing area.  A new insurance card will be issued to you with the new address which should be used while out of the Peoria NetWork.  Do not discard the insurance card for the Peoria NetWork.  When you return home, call Caterpillar to report your address change so that you can again be coded for the Peoria NetWork.

 

v     If you are traveling full-time, all non-emergency treatment will be penalized at 70% originally.  When you receive an explanation of benefits with penalized benefit payments, a phone call to United Healthcare to report that you are traveling should cause them to reprocess the claim properly to resolve the issue.
