1.) 1910.179(j)(3) Periodic inspection. Complete inspections of the crane
shall be performed at intervals as generally
defined in paragraph (j)(1)(ii)(b) of this section, depending upon its
activity, severity of service, and environment, or as specifically indicated
below. These inspections shall include the requirements of paragraph (j)(2)

1, 3) 1910.215(a)(4) Work rests. On offhand grinding machines, work rests
shall be used to support the work. They shall be
of rigid construction and designed to be adjustable to compensate for wheel
wear. Work rests shall be kept adjusted closely to the wheel with a maximum
opening of one-eighth inch to prevent the work from being jammed between the
wheel and the rest, which may cause wheel breakage. The work rest shall be

Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the
operator stands in front of the opening, shall be constructed so that the
peripheral protecting member can be adjusted to the constantly decreasing
diameter of the wheel. The maximum angular exposure above the horizontal
plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this
section shall never be exceeded, and the distance between the wheel periphery
and the adjustable tongue or the end of the peripheral member at the top
shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21,

5(a)(1) |
Section 5(a)(1) of the Occupational Safety and Health Act of
1970: The employer did not furnish employment and
a place of employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to employees in
that press operators were exposed to the hazard of being struck by the mold
drawers: The mold drawers for the #4 Hydraulic Press in the Mounts Area were
not adequately protected from rolling out of their tracks as the employer was
not requiring the utilization of two stops on each mold drawer on the
operator side of the machine when it was evident that mold drawer stops
frequently fail subjecting employees to the hazard of falling mold drawers.
All four of the mold drawers had only one mold stop instead of two on the
operator end of each drawer, and the top drawer that came loose from the
press had the one mold drawer stop hanging from one bolt. On or about
December 13, 2005, at 2416 Mid-America Industrial Drive, Boonville, Missouri,
a press operator received fatal injuries when the press operator was struck
by the top mold drawer when it came out of the rails at the end of the press.
Among other methods, one feasible and acceptable method to correct this
hazard is to ensure the following: 1)Require the use of two mold stops,
instead of just one, for each drawer. 2)Design a better mold stop for the end
of the press to ensure that the tray does not come off the end of the rails
when the press is being utilized. 3)Design and install a redundant stopping
system for mold drawers. Abatement certification is required for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition,
documentation demonstrating that abatement is complete must be included with
your certification. This documentation may include, but is not limited to,
evidence of the purchase or repair of the equipment, photographic or video
evidence of abatement, or other written records. |

Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the
operator stands in front of the opening, shall be constructed so that the
peripheral protecting member can be adjusted to the constantly decreasing
diameter of the wheel. The maximum angular exposure above the horizontal
plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this
section shall never be exceeded, and the distance between the wheel periphery
and the adjustable tongue or the end of the peripheral member at the top shall
never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and

Section 5(a)(1) of Public Law 91-596, the Occupational Safety
and Health Act of 1970: The employer did not
furnish employment and a place of employment which were free from recognized
hazards that were causing or likely to cause death or serious physical harm
to employees, in that employees were exposed to: the hazard of cranes being
overloaded, and/or loads being dropped, due to the absence of rated loads for
"Below-the-Hook" Lifting Devices, i.e., not having been
conspicuously displayed on the device(s). (a)Production coating lines,
Northwest section of plant - There were no rated load markings visible on the
main structure of, nor any identification plates which listed: a. the
manufacturer's name and address, b. the serial number of the device, c. the
lifter's weight (if over 100 lbs), d. its rated load on the "Below-the-Hook"
Lifting Devices attached to overhead cranes running above and over: (1) the
Phosphate Coating Dip-tank Line, and (2) the Rack Plating Dip-tank Line.
[Reference Section 20-1.2.1(a) & (b) of The American Society of
Mechanical Engineers Safety Standard for Below-the-Hook Lifting Devices, ASME
B30.20a-1996.] Among other methods, one feasible and acceptable abatement
method to correct this hazard is to: (1) stencil a clearly visible load
rating on each "Below-the-Hook" lifting device, and (2) affix an
identification plate to each "Below-the-Hook" lifting device which
includes the information required in points a-d above. |

(a) Every employer shall report immediately by
telephone or telegraph to the nearest District Office of the Division of
Occupational Safety and Health any serious injury or illness, or death, of an
employee occurring in a place of employment or in connection with any employment.
Immediately means as soon as practically possible but not longer than 8 hours

death or serious injury or illness. If the employer can demonstrate that

Section 5(a)(1) of the Occupational Safety and Health Act of
1970: The employer did not furnish employment and
a place of employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were exposed to: In Department 8894, Grader Tire Build Area,
employees were exposed to the explosive separation of the wheel components
while mounting tubeless tires on multi-piece (three piece) grader rims and
the employer did not ensure that the employees were protected from the
hazards during the operation. Among other methods, feasible and effective
means of abatement to correct this hazard can be to provide a restraining
device or other protective system in order to protect employees during
inflation, inspection, and insertion of the valve core, update the product
and process specifications and safe job procedures, and providing training to
all employees on the hazards and operating procedures to be followed. ***THIS

Section 5(a)(1) of the Occupational Safety and Health Act of
1970: The employer did not furnish employment and
a place of employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were exposed to: On or about October 13, 2006 in buildings BB
and DD, employees working in the hot engine test cells were exposed to
contact with fuel, oil, coolant, air lines and other engine parts while
obtaining oil samples during the different test cycles conducted on running
engines. Employees have received thermal burns from leaking lines spraying
oil and coolant and contact with hot engine parts. Among other methods, a
feasible and acceptable method to correct this hazard is to install remote
cameras inside the test cells at locations which will allow employees to
monitor engine testing safely from outside the test cells and install an
automated oil sampling system to eliminate employee exposure to operating
engines to obtain oil samples. *** THIS ITEM REQUIRES DOCUMENTATION OF
